In case of minor children of unmarried single parents, along with filling up the Annexure 'D' form, the unmarried parent also submits an affidavit as per Appendix-12.

For issuing passport of a child born through surrogacy, a declaration has to be given by the genetic parents of the applicant and for that the guardian has to fill up the Annexure 'I' form.
Declaration of Parent/Guardian for Minor Passports (Annexure "D")
Annexure D form is filled while issuing passport of minor children, this is for all normal cases where both the parents have to sign the Annexure D.Both parents have to be present when issuing passports of minor children. This Annexure also applies to single parents who have full custody of the child.

When issuing a passport for a minor, if for some reason only one of the parents is present or one parent has not given consent, then Annexure C form is filled and along with it Annexure D form is also filled.
Those who are high ranking government officials have to fill Annexure A form as their identity proof. This is given by their department to show that their department has no objection in making their passport. It can be called a no objection certificate.
