Annexure D form is filled while issuing passport of minor children, this is for all normal cases where both the parents have to sign the Annexure D.Both parents have to be present when issuing passports of minor children. This Annexure also applies to single parents who have full custody of the child.

Annexure J

Joint Declaration in lieu of Marriage Certificate (Annexure "J")

Annexure J is a signed joint photo declaration in lieu of a marriage certificate, used if one of the parties does not have a marriage certificate Or if the spouse's name is not added in the old passport, then a joint declaration has to be given in its place through Annexure J. A joint photo of the couple is attached to it, and it is necessary for both of them to come on the day of the appointment.
