Joint Affidavit To Be Submitted By Both Biological Parents Of Minor Child Born Outside Of Wedlock
In case of minor children of unmarried single parents, along with filling up the Annexure 'D' form, the unmarried parent also submits an affidavit as per Appendix-12.
