How do I Apply for a Passport if my name has changed after marriage?

If your name or surname is different before marriage and changed after marriage, you can apply under the normal passport category, as this cannot be processed under the Tatkal scheme.

Documents required for Name Change

  • A public notice of the name change must be published in two major newspapers.
    • For a name change advertisement, the applicant must first obtain an affidavit from the court.
    • Based on this affidavit, the applicants can then publish a name change advertisement in the newspapers.
  • Marriage Certificate
    • In the absence of a marriage certificate, both husband and wife must visit the passport office on the appointment day and fill out the Annexure J form.
  • Identity Proof/Address Proof (PAN Card/Voter ID/Aadhaar Card)
  • Proof of birth (PAN Card/Birth Certificate)
  • Education Qualification documents.

Application Process

  • Visit the Passport Seva Portal.
  • Register if you are not a registered user.
  • Log in with the details received.
  • Click on the 'Apply for Fresh/Reissue of Passport'.
  • Fill in the details in the application form.
    • New Application can directly add the spouse's name.
    • If applying for renewal, click on the "Change in Existing Personal Particulars" and then select Spouse Name.
  • Enter your personal details.
    • Enter your old name (if applicable)
    • Fill in all personal details (name, Address, DOB, etc)
    • Upload the required documents and submit the form.
  • Pay the application fees.
    • Fees can be paid via Net Banking /Credit Card/DebitCcard, or UPI.
  • Choose a Passport Seva Kendra (PSK)/Post Office Passport Seva Kendra (POPSK) and schedule your appointment.
  • Visit the PSK/POPSK on the scheduled date and time with all original documents.
  • Officials will verify your documents. Once successfully verified, your application will be forwarded for police verification.
  • After a successful police report, the RPO will issue your passport.
  • The passport will be sent to your address given in the application form via speed post.
  • After documentation at the PSK/POSPK, it usually takes 15 to 20 days for the passport to be issued.
