Additional Documents for Minor Children

Yes, an Indian minor below 18 years of age must submit the following additional documents when applying for a passport.

  • Birth Certificate (as proof of date of birth)
  • Aadhaar Card (if available for address proof)
  • Current Address Proof (Mother/ Father Aadhaar Card/ Voter ID)
  • Valid passport of both parents or at least one.
  • Annexure 'D' declaration form (duly signed by parents, giving consent for the child's passport)
  • Annexure 'C' declaration form (if one of the parents is absent)
  • Recently clicked passport-size photograph with a white background in a standard size of 4.5 cm x 3.5 cm (for minor applicants below 4 years of age)

Additional Documents as per the Applicable Case

  • Single Parent/ Divorce/ Separated parents
    • Custody documents of the minor child.
    • Annexure 'C' declaration form (if any of the parents is absent or in custody)
  • Adopted Child
    • Adoption paper (if the child is legally adopted)
  • If one parent has the child's custody
    • Custody documents
    • A declaration order issued by the court.
  • In case of the death of a parent
    • Death Certificate of the deceased parent.
  • If one parent does not give consent
    • Annexure 'C' Declaration form.
    • Court Declaration or order (in case of divorce or separation)
