Additional Documents for Minor Children
Yes, an Indian minor below 18 years of age must submit the following additional documents when applying for a passport.
- Birth Certificate (as proof of date of birth)
- Aadhaar Card (if available for address proof)
- Current Address Proof (Mother/ Father Aadhaar Card/ Voter ID)
- Valid passport of both parents or at least one.
- Annexure 'D' declaration form (duly signed by parents, giving consent for the child's passport)
- Annexure 'C' declaration form (if one of the parents is absent)
- Recently clicked passport-size photograph with a white background in a standard size of 4.5 cm x 3.5 cm (for minor applicants below 4 years of age)

- Single Parent/ Divorce/ Separated parents
- Custody documents of the minor child.
- Annexure 'C' declaration form (if any of the parents is absent or in custody)
- Adopted Child
- Adoption paper (if the child is legally adopted)
