To reissue a lost passport, applicants must provide some additional documents.

• A Copy of the old passport.
• FIR report lodged at the police station, with GD number.
• Address proof.
• Birth Date Proof (PAN Card/Birth certificate, etc.)
• Marriage Certificate (if applicable)
• Annexure 'F' (applicant's declaration form for obtaining a passport in lieu of lost passport)

Note: It is to be understood that the Lost passport cannot be reissued under the Tatkal Scheme.
